Who this guide is for

Section actions
  • You if you have been diagnosed with a chronic disease
  • If you want to understand how to get ALD and 100%
  • You if your doctor has mentioned a "ALD care protocol"

To be prepared in advance

Section actions
  • Diagnosis and medical opinion of specialist
  • Care protocol form (Cerfa), completed by the doctor
  • Medical history and examinations

Step-by-step procedure

Section actions
  1. 1Confirm the diagnosis in a specialist: the doctor completes the ALD treatment protocol (Cerfa 11626) describing the disease and the necessary care.
  2. 2The doctor sends the protocol to the CPAM (or you drop it yourself). CPAM investigates the case through its medical consultant.
  3. 3The CPAM attributes the ALD (usually on the ALD 30 list — diabetes, cancer, HIV; or ALD off-list for other chronicles, requiring additional evidence).
  4. 4In case of favourable opinion: the CPAM issues an ALD notification with validity period (often renewable). You benefit from the exemption from the user ticket — 100 % refund for care related to the ALD.
  5. 5Use the Vital Card: care and medicines in ALD will be in paid third party (free or stay only for overtakings).
  6. 6Renewal: 2 at 3 months before expiry, the doctor updates the protocol, the CPAM prolongs.
  7. 7LALD does not cure the disease; it removes the financial burden of treatment. Non-ALD-related care is normally reimbursed (70 %, remains with mutual care).
  8. 8It also provides for prolonged sick sentences (3 years of IJSS) and protection against dismissal for certain judgments.

Common mistakes

Section actions
  • Do not ask the doctor for the ALD protocol — loss of 100%.
  • Believing that ALD covers ALL care. Only those of the protocol (related to disease).
  • Do not renew the ALD — the coverage expires.
  • Do not present the Vital Card with current ALD — pay in advance.
  • Hide the ALD from the employer — loss of protection in stoppage.
